H3: product analysis of 106f-071-100, lotno:19481-01.Damage location details: no damages were found with the rist-071 catheter hub.The rist-071 catheter body was found to be kinked at the strain relief ~4.0cm from the proximal end of the catheter hub.The strain relief was removed, and the catheter body was found broken at the damaged (kinked) location. The rist-071 catheter distal tip was found to be in good condition.Testing/analysis: the rist-071 catheter was flushed, water leaked from the catheter kink/broken location. Conclusion: based on the device analysis and reported information, the customer¿s ¿catheter kink/damage¿ and ¿catheter leak at hub/strain relief¿ reports were confirmed as the returned rist-071 catheter was found kinked/broken and leaking. It is possible that the rist-071 catheter became kinked due to handling, insertion of the catheter, or manipulation/advancement of the catheter within the patient's vasculature, subsequently leaking.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
