Event or Problem Description
Information was received from a manufacturer¿s representative (rep) who reported a patient underwent neurostimulator replacement today.Pre-operatively, impedances were checked, and all were within range.Upon connecting existing leads/extension to the new neurostimulator the left lead had out of range impedances. The physician opened the pocket, removed the extension tail and wiped it off, ensured neurostimulator port was clear, connected extension back to ins, tightened the setscrew and placed the neurostimulator in the pocket to test impedances.Impedances showed the right and left lead were now all completely red/out of range.Impedances were checked several more times and impedances remained out of range on both leads. The physician disconnected extensions/lead from the neurostimulator and placed them in the original neurostimulator and tested impedances and everything was normal. At this point, the physician decided to open a new neurostimulator but again, once everything was connected, impedances were still showing as a problem on both the leads. The physician decided to close the patient with the impedance issues still present on the second new neurostimulator. The issue was not resolved through troubleshooting. The rep was going to try and collect the original neurostimulator and the first neurostimulator that was used as a replacement for analysis.
