Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary : according to the information received, ¿it was reported that the fixation pin got stuck in hole of the distal cutting block. Can't get it out.Need new cutting block¿.The product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample found an unknow fixation pin jammed and unable to be removed from the cut block. It is worth mentioning that all the threaded shaft of the pin passed through the cutting block and only the proximal portion was jammed. However, the sigma hp uni distal cut block presents no signs of damage nor cosmetic defects that could have contributed to the reported event. The observed condition of the device was consistent with a random component failure that may have been caused by exposure to unintended and heavy forces, such as, but not limited to an off axis insertion and drilling the pin beyond the intended purpose.A dimensional inspection and functional testing were not performed as it is not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of the sigma hp uni distal cut block would contribute to the complained device issue.Based on the investigation findings, the potential cause is traced to unintended user error, and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot : a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code (pg0609) provided is not a valid finished goods lot#.
