Event or Problem Description
It was reported that on (b)(6) 2023, a 5mm by 4mm amplatzer piccolo occluder was chosen for implant for a 2 week old, 1.2kg patient with the following patent ductus arteriosus (pda) dimensions: minimal diameter of 3.4mm, diameter at aortic ampulla of 3.6mm, and ductal length of 12mm.A series of follow up echocardiograms showed a stable left pulmonary artery (lpa) gradient of approximately 10/12mmhg (15-17m/sec).On 07 september 2023, the echocardiogram demonstrated severe lpa obstruction. The patient was referred for further intervention. The patient status was reported as stable.

Additional Manufacturer Narrative
An event of severe lpa obstruction and device migration was reported.A review of the ductal measurements as reported from the field was performed.Per the sizing table in the instructions for use, the recommended size devices for patients >2kg have a maximum duct length of 8mm, and therefore the 12mm patent ductus arteriosus length of the reported event falls outside recommended sizing.A returned device assessment could not be performed as the device was not returned for analysis. The device history record was reviewed to ensure that each manufacturing and inspection operation was performed and the product met all specifications.Based on the information received, the cause of the reported incident could potentially be related to the user not following the recommended sizing chart provided in the ifu. There is no indication of a product quality issue with respect to labeling design or manufacturing of the device.
