Reported event: an event regarding abnormal ion level and wear involving an accolade stem was reported. The event of wear was confirmed.Method & results: product evaluation and results: material analysis, visual, functional and dimensional inspections could not be performed as the device was not returned.Clinician review: a review with a clinical consultant indicated" this female patient underwent a primary total hip arthroplasty with cementless components in 2013 and according to the product summary developed elevated ion levels that require revision surgery in 2022.Some trunnionosis was found.I can confirm that the patient underwent the primary and revision hip arthroplasties since i was able to review the operation reports.I did not have access to any laboratory values.I cannot determine the root cause of this event with certainty. The causes of trunnionosis are multifactorial including but not limited to surgical technique, especially in the preparation and insertion of the femoral head on the trunnion. In this case the surgeon stated that he did clean and dry the trunnion. Also contributing factors can be patient factors such as activity level and bmi, and implant factors." -product history review: review of the device history records indicate devices were manufactured and accepted into final stock with no relevant reported discrepancies.-complaint history review: there have been no other similar events for the lot referenced. Conclusions: it was reported that the patient was revised due to elevated metal ion levels.A review with a clinical consultant indicated" this female patient underwent a primary total hip arthroplasty with cementless components in 2013 and according to the product summary developed elevated ion levels that require revision surgery in 2022.Some trunnionosis was found.I can confirm that the patient underwent the primary and revision hip arthroplasties since i was able to review the operation reports.I did not have access to any laboratory values.I cannot determine the root cause of this event with certainty. The causes of trunnionosis are multifactorial including but not limited to surgical technique, especially in the preparation and insertion of the femoral head on the trunnion. In this case the surgeon stated that he did clean and dry the trunnion. Also contributing factors can be patient factors such as activity level and bmi, and implant factors." no further investigation for this event is possible at this time.If devices and/or additional information become available to indicate further evaluation is warranted, this record will be reopened.
