Event or Problem Description
Physician attempted to use a visipro balloon expandable stent with a 7x10cm non-medtronic sheath and a 0.035/260cm non-medtronic guidewire during treatment of a calcified lesion in the patient¿s left proximal renal artery. No vessel tortuosity reported. The lesion was moderate to heavy calcified left renal artery, diseased at proximal vessel and fairly straight segment.Embolic protection was not used. There was no damage noted to packaging (i.E.Shelf carton, hoop/tray).There were no issues noted when removing the device from the hoop/tray.The device did not pass through a previously-deployed stent.Resistance was encountered when advancing the device. It was reported on (b)(6) 2023, during an abdominal angiogram with planned renal artery stent placement, the visipro stent was unable to cross the lesion.During attempt to remove stent from the body, the balloon-mounted visipro stent slipped off the delivery system. After removing delivery system from the body, the physician attempted to use a 7fr 27-45mm non-medtronic snare to retrieve stent but it was unsuccessful. The snared stent was too big to remove through the sheath and the access was lost in order to remove the stent.A femoral artery cutdown and repair was performed.Patient was transferred to post-anaesthesia care unit in stable condition. On (b)(6) 2023, patient was brought back to cath lab and a brachial cutdown was performed.Angioplasty was performed using a pre-dilation 3.0x20mm non-medtronic balloon and a 5.0x28 non-medtronic coronary stent to renal artery, patient then was transferred back to unit in stable condition and discharged home on (b)(6) 2023 without any further complications or known procedures.
