The reported event could be confirmed since the device returned and is indeed broken. The devices returned include a long gamma nail, a lag screw, a set screw and a distal locking screw. The catalog and lot numbers reported could be confirmed. The nail was indeed confirmed to be broken in two at the level of the lag screw hole.On the proximal broken part, on the inside wall of the lag screw hole, misdrilling marks can be noticed, indicating that the nail was hit by the drill during the preparation of the lag screw hole.A fracture analysis of the broken bridges shows clear rest lines going from the lateral to the medial side, highlighted in red below. These rest lines are clear indicators of a fatigue fracture. The fracture most probably started when the drill hit the nail during the preparation of the lag screw hole, introducing a fracture that propagated during the use of the product, and leading to a total breakage of the two parts of the nail.Based on investigation, the root cause was attributed to be user related. The misdrilling which happened during the preparation of the lag screw hole preparation is the most probable root cause of the fatigue fracture of the nail.A review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required at this time.A review of the labeling did not indicate any abnormalities. No indications of material, manufacturing or design related problems were found during the investigation. If the device is returned or if any additional information is provided, the investigation will be reassessed.
