Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary : during surgical procedure at the time of milling in the calcar i proceed to pass one of the calcar regularizers ref 257004100 and during its operation this is dull, i proceed to pass the other one that is available in the equipment ref (b)(4) and it does not serve to carry out the milling however this neither has edge. Therefore, the specialist manifests his disagreement since the instrument does not work and this remodelling had to be done manually with gouge and saw blade which delays the procedure about 10 minutes more. The device associated with this report was returned to depuy synthes for evaluation.Visual analysis of the device found rounding of cutting edges. It is not unreasonable that the condition identified in visual analysis would contribute to a dull condition. Therefore we are able to confirm dullness with damage observed, as this kind of evidence indicates repeated use of the device. The lifecycle requirements of the device are event related and depend on the use in clinical practice, the device must be properly inspected prior to each surgical use.Refer to the device/country specific ifu for information related to end of life, reprocessing instructions, and inspection procedures.A dimensional inspection was not performed since it was not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of summit calcar planer-large would have contributed to the complained issue.Based on the investigation findings, the potential cause is traced to end of life, and it has been determined that no corrective and/or preventative action is required. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot : a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code ay0306 (march, 2006) provided is not a valid finished goods.
