The lens was returned partially inserted into the loading area of a cartridge.Viscoelastic was observed on the lens.No viscoelastic was observed in the cartridge. The cartridge had evidence that it was placed into a handpiece. The lens was removed from the cartridge and cleaned with klrp for further evaluation. The lens was foldable using folding tweezers and unfolded with no abnormalities observed. No stiffness was felt.Product history records were reviewed and the documentation indicated the product met release criteria. The account used a qualified cartridge and handpiece with a non-qualified viscoelastic. The root cause for the reported loading issues appears to be related to a failure to follow the ifu.The lens was returned partially inserted into the cartridge loading area.Viscoelastic was observed on the lens.No viscoelastic was observed in the cartridge. The ifu instructs to completely fill the cartridge with ovd immediately prior to loading and delivery of the lens.Do not attempt to load the lens without adequate ovd in the device. Not adequately filling the device with viscoelastic will result in inadequate coverage of lens and the lens fold path with ovd, which may result in damage. The lens was evaluated and no "stiffness" was felt.It is unknown if the delivery system was being used at the recommend temperature. The ifu instructs to use the cartridge at operating room temperatures between 18° c (64° f) and 23° c (73° f).In addition, a non-qualified viscoelastic was indicated.Company foldable iols are qualified for use with a company qualified delivery system (handpiece and cartridge) and ophthalmic viscosurgical device (ovd) combination. The ifu instructs that a company qualified delivery system and viscoelastic combination should be used. The use of an unqualified combination may cause damage to the lens and potential complications during the implantation process. The manufacturer internal reference number is: (b)(4).
