It was reported that the procedure was to treat a lesion in the iliac artery with heavy calcification and no tortuosity. The 8.0x60mm absolute pro vascular self-expanding stent system (sess) was advanced without resistance and deployed. The delivery system was removed with a little resistance due to the anatomy and the stent implant was noted to be broken having separated into two pieces in the target lesion.A snare device was used to remove the broken stent and nothing was left in the anatomy. An omnilink elite stent was used to complete the procedure. There was no adverse patient sequelae and there was no clinically significant delay in the procedure. No additional information was provided.

Additional Manufacturer Narrative
The device not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents from this lot.The investigation determined the reported difficult to remove appears to be related to circumstances of the procedure as it is likely that during device removal resistance was met with the heavily calcified anatomy.Additionally, it is possible that during device removal interaction with the deployed stent resulted in the reported stent material separation; however, this cannot be confirmed. The investigation determined a conclusive cause for the reported material separation cannot be determined. There is no indication of a product quality issue with respect to manufacture, design or labeling.
