Complaint allegation is confirmed.Upon visual inspection, it was noted that the threaded shaft was broken off.It was observed the activation sleeve was inside the telescoping lag screw, 10.5mm x 95mm.The slots are inside the barrel, and strong abrasion marks are noted around the shaft.Functional testing was not performed due to the damage to the device. The most likely cause of this failure is attributed to user error due to the activation sleeve had not been removed from the telescoping lag screw.Per surgical technique, to allow up to 10mm postoperative compression in the telescoping lag screw, remove the activation sleeve by inserting the activation tool through the sheath into the lag screw and turning counterclockwise. The activation sleeve will retain to the tip of the activation tool once disengaged from the lag screw.Remove the lag screw sheath.Preoperative and operating procedures, including knowledge of surgical techniques and proper selection and placement of the device, are important considerations in the successful utilization of this device. The appropriate arthrex delivery system is required for proper implantation of the device.
