Corrected information: d1, d4, g4, h4 = on (b)(6) 2024, it was determined that the device that was returned to cook was not the rpn of the device reported by the customer; however, the device damage found during the dfa was consistent with the damage reported by the customer. Therefore, it was determined that the device that was returned to cook was packaged incorrectly (the thscf-35-260-3-aes package contained the wrong device).Sections d1 and d4 have been updated to unknown.Section g4 and h4 are also unknown.Summary of event: as reported, upon opening the device package prior to use during a transcatheter aortic valve implantation procedure, via access in the right femoral vein, a safe-t-j amplatz extra-stiff support wire guide's coils were broken and "popping out" approximately two centimeters from the tip.The device was not used.Another device of the same type, from an unknown lot, was used to complete the procedure.Per the user, wires are usually soaked in saline after being removed from the wire holder. The patient did not require any additional procedures due to this occurrence.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.Investigation evaluation: reviews of the complaint history, device history record (dhr), instructions for use (ifu), manufacturing instructions, and quality control procedures were conducted during the investigation.A visual inspection of the complaint device was also conducted. The prior-to-use complaint device was returned to cook for investigation. The coils were off-set approximately 2.5-centimeters from the apex of the curve, and the inner mandril protruded from the coils.Upon evaluation of the returned device, it was determined that the rpn of the device that was returned did not match the rpn that was reported by the customer; however, the damage found during the device failure analysis was consistent with the damage reported by the customer. Therefore, it was determined that the device that was returned to cook was packaged incorrectly (the thscf-35-260-3-aes package contained the wrong device).A document-based investigation evaluation was performed. The lot number of the wire that was returned is unknown, as the wire was within the wrong package. As such, the device history record and complaint history could not be reviewed.A review of the device master record (dmr) concluded that sufficient inspection activities are in place to identify this failure mode prior to distribution. The information provided upon review of the dmr, ifu, and investigation of the returned device suggests that there is evidence the device was manufactured out of specification; however, there is no evidence of non-conforming devices in-house or in the field.Based on the information provided and the results of the investigation, cook has concluded that a quality control deficiency contributed to this event.Responsible personnel were notified. The risk analysis for this failure mode was reviewed and no additional escalation was required. The appropriate personnel have been notified and cook will continue to monitor for similar events. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
