Event or Problem Description
Information was received from a healthcare provider regarding a patient who was implanted with an implantable neurostimulator (ins) for fecal incontinence and urge incontinence.Their trial start date was (b)(6) 2023.It was reported that the lead was broken. The lead tip wouldn't advance and appeared to have gotten stuck in the needle. The cause was unknown. This happened to a second lead as well. The issue was resolved.Additional information was received from the manufacturer representative (rep). They stated that the pne needle was inserted into the foramen of the patient and the physician tried to put the lead into the needle and the tip got floppy. The manufacturer representative (rep) believed it was physician¿s error as they pulled it a bit without knowing so the stylet wasn¿t all the way in it and therefore the tip of the lead was too soft to advance through. The physician did this twice. The lead never touched the patient; it was in the hub of the needle. They did not know the lot number or patient weight.
