Product complaint # (b)(4).Investigation summary: according to the information received, "[defective device. Both items are normally held together by a small pin and this wasn't a case.No patient consequences.]" the product was not returned to depuy synthes, however photos were provided for review. The photo investigation revealed that the device 230772003, delto pectoral cutting guide had missing stopper and platinum screw and parts (support and slider) were fell apart due to broken weld. The observed condition of the device was consistent with a component failure that was caused by exposure to unintended forces like usage of excessive force in a prying motion while trialing.Since the device was not returned, a dimensional inspection cannot be performed. The overall complaint was confirmed as the observed condition of the device 230772003, delto pectoral cutting guide would contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the device lot number is unknown, therefore a device history review could not be performed. If the lot/serial number becomes available, the record will be re-assessed.

Product complaint # (b)(4).Investigation summary : defective device. Both items are normally held together by a small pin and this wasn't a case.See picture in the attachments. No patient consequences. The product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that delto pectoral cutting guide had the butee pin broken. The broken fragment was not returned.Based on the damage observed it is reasonable to conclude that the device it unable to hold or retain as intended. The allegation can be confirmed. The observed condition of the device was consistent with a component failure that was caused by exposure to unintended forces like usage of excessive force in a prying motion while trialing.A dimensional inspection was performed for the delto pectoral cutting guide was unable to be performed due to post manufacturing damage. The overall complaint was confirmed as the observed condition of the delto pectoral cutting guide would contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot : the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A records evaluation (mre) was not performed.
