Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: the femoral broach does not fit correctly into the corresponding handle or into the test necks.Maybe the fitting has bent.Something that has happened with other similar instruments after a few uses. The product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the post of the broach corail amt 9 presents deep scratches all over the surface.Additionally, nicks and signs of deformation were found on the distal portion of the post.A functional test performed with a mating broach handle (952210500f) revealed that the broach was not able to be assemble. Most likely due to the observed condition of the post. The reported complaint condition was able to be replicated. The observed condition of the device is consistent with a combination of heavy usage when interacting with the device and unintended impaction an prying forces while the broach is not fully seated to the mating handle. The overall complaint was confirmed as the observed condition of the broach corail amt 9 would contribute to the complained device issue.Based on the investigation findings, the potential cause is traced to unintended user error, and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
