Additional Manufacturer Narrative
Pma/510(k) # k163468.Device evaluation: the evo-22-27-12-d of lot c1891105 involved in this complaint was returned for evaluation, with its original opened packaging. With the information provided, a physical examination and document-based investigation was conducted. The device related to this occurrence underwent a laboratory evaluation on the on 22nd nov 2023.On evaluation of the device, the following was observed: visual inspection: red safety tab returned separately.Device returned in protective tubing.Stent partially deployed on return.Red shuttle on the 01st dimple.Directional button in the deploy position.Safety wire appears to be slightly pulled back into delivery system.Kink noted on the outer sheath measuring approximately 7.6 cm from end of handle.Functional inspection: handle actuating fine for deployment and recapture.Unable to remove the safety wire, due to its position within the delivery system.Stent deployed.Stent intact.Handle opened and all inner component intact.Safety wire removed. It may be noted that while evaluating the device, the purple luer accidently snapped and detached from the back of the delivery system.Photographic evidence supplied by the customer showed the box of the evolution device and the device with stent partially deployed.Manufacturing records: prior to distribution all devices are subjected to a visual inspection and functional inspection to ensure device integrity.A review of the manufacturing records of lot c1891105 did not reveal any discrepancies that could have contributed to this complaint issue.Review historical data: the review of relevant manufacturing records of lot number c1891105 confirms the failure mode has not previously occurred for this work order.Instructions for use and/label: the instructions for use, ifu0053 that accompanies this device states ¿¿ visually insect with particular attention to kinks, bends and breaks. If an abnormality is detected that would prohibit proper working condition, do not use.¿¿ there is no evidence to suggest that the customer did not follow the instructions for use.Image review: an image was not returned for evaluation.Root cause analysis: a definitive root cause for the customer complaint could not be determined as circumstances of use cannot be replicated in the laboratory.A possible root cause of the kinked flexor observed in the lab is most likely due tortuous anatomy/tight stricture leading to a build up of pressure and therefore causing the issue deploying the stent as reported. It should be noted that the stent deployed with no issues and was intact when evaluated. It was noted that the safety wire could not be removed but this may have been due to the kinking and build up of pressure leading to it getting caught on the delivery system.Confirmation of complaint: complaint is confirmed based on customer and/or rep testimony.Summary of investigation: according to the customer the stent could not be released.Confirmed quantity of 1 device, confirmed used.A definitive root cause for the customer complaint could not be determined as circumstances of use cannot be replicated in the laboratory.A possible root cause of the kinked flexor observed in the lab is most likely due tortuous anatomy/tight stricture leading to a build up of pressure and therefore causing the issue deploying the stent as reported. It should be noted that the stent deployed with no issues and was intact when evaluated. It was noted that the safety wire could not be removed but this may have been due to the kinking and build up of pressure leading to it getting caught on the delivery system.According to the initial report, the patient did not experience any adverse effects due to this occurrence.Complaint is confirmed based on customer and/or rep testimony.Complaints of this nature will continue to be monitored for similar events.
