Medtronic received a report that the distal end of the pipeline would not opened and appeared stuck. The pipeline was positioned in a bend in the middle section.More than 50% of the pipeline was deployed when it failed to open and the pipeline was resheathed more than 2 times. The pipeline was not used for an off-label use.The pipeline and any accessories were prepared as indicated per the instructions for use (ifu). No patient symptoms or further complications were reported as a result of this event. The patient was undergoing surgery for treatment of a wide neck ophthalmic, unruptured internal carotid artery (ica) aneurysm. The landing zone artery size measurements were 4.5mm distally and 4.7mm proximally. It was noted the patient's vessel tortuosity was moderate.Dual antiplatelet therapy (dapt) was administered. The post procedure angiographic results were good.Ancillary devices include a phenom plus guide catheter, phenom 27 microcatheter, and balt coils.

Product analysis # (b)(4): equipment used: video inspection system (m-81805), camera (panasonic lumix dmc-zs5) as found condition: the pipeline flex shield was returned for analysis inside a sealed biohazard bag and a shipping box.Damaged location details: the tip coil was observed to be damaged. The distal and proximal dps restraints and dps sleeves were found to be intact, with no signs of damage. No defects were found on the re-sheathing marker or with the proximal bumper. The distal hypotube and ptfe shrink tubing were intact, with no signs of elongation and damage. The braid¿s distal end was not opened and frayed, while the proximal end was opened and frayed. The pushwire was tied in a knot, and kinks were observed along its length. No other anomalies were noted.Testing/analysis: n/a conclusion: the reported information and device analysis confirmed the customer¿s complaint of ¿failure to open¿ as the braid¿s distal part was not opened and frayed. However, the cause of the failure to open could not be determined.Possible causes include vessel tortuosity, the user deploying the braid in the vessel bend, and damaged braid. The customer reported that the vessel tortuosity was moderate, therefore ruling out the vessel tortuosity as a potential cause. The user deploying the braid in the vessel bend and damaged braid may have contributed to the failure to open. The braid can be damaged due to re-sheathing more than two times, over-manipulation, deployment technique, delivering/retracting delivery wire against resistance, or deploying/re-sheathing against resistance can also contribute to damaged braid. The cause of the ¿failure to open¿ could not be determined. In addition, the pushwire was returned tied in a knot, and the damages(kinks) observed along the pushwire could have been due to handling.Per the instructions for use (ifu): ¿re-sheathing the pipeline flex embolization device with shield technology more than 2 full cycles may cause damage to the distal or proximal ends of the braid.¿ medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
