Event or Problem Description
Additional information was received that per the physician, the cause of the regurgitation was due to the pulling of the coil into the dcs, and the non-medtronic sheath was near a chordal attachment causing the tear.Per the physician, the dcs caused/contributed to the tear.It was further reported that treatment was not provided for the tear, and the patient¿s symptoms were improving.Per the physician, the distance from the tricuspid valve to pulmonary valve was shorter than normal, which contributed to the tear.Additionally, it was noted that the patient had mild tricuspid regurgitation prior to implant, and the flailed tricuspid chordae tendineae was not present prior to implant. No additional adverse patient effects were reported.

Event or Problem Description
Additional information was received that at the time of the procedure, the physician thought both the dcs, and non-medtronic sheath were clear of the tricuspid valve, and tricuspid valve chordae tendineae. When looking at the angiogram¿s post-procedure, the physician concluded that the chordae was likely caught in between the dcs and sheath. This was concluded to be because the right ventricular outflow tract was shorter than it was thought to be.Per the physician, the valve did not cause or contribute to the tear.Additionally, the patient did not have any reported symptoms, only increasing regurgitation that showed slight improvement on the echocardiograms the following day.No additional adverse patient effects were reported.
