Investigation ¿ evaluation: it was reported that the wire guide from a wayne pneumothorax set broke inside a patient during an intervention.A photo provided by the customer shows the wire guide unraveled.A computed tomography (ct) scan was performed to confirm no portion of the device was retained in the patient; results are pending. The patient's condition was noted to be "good".Reviews of documentation including the complaint history, device history record (dhr), quality control procedures, specifications, and instructions for use (ifu), as well as a visual inspection, functional test, and dimensional verification of the returned device, were conducted during the investigation. One used wire guide, dilator, straightening obturator, and catheter was returned to cook for evaluation.Upon visual inspection, the straight end of the wire guide was noted to be unraveled. The catheter was checked with a.038¿ wire guide which passed through with only slight resistance due to the biological matter. The device failure analysis did not identify any non-conforming material.Dimensional analysis confirmed that the device and components were manufactured to the correct specifications and tolerances.Additionally, a document-based investigation evaluation was performed.A review of the device master record (dmr) concluded that sufficient inspection activities are in place to identify this failure mode prior to distribution.A review of the dhrs for the reported complaint device lot and the related subassembly lots revealed no relevant non-conformances.A database search for complaints on the reported lot found no additional complaints reported from the field.Cook also reviewed product labeling. The product ifu, [c_t_waynemod_rev5] ¿wayne pneumothorax set for seldinger placement,¿ provides the following information to the user related to the reported failure mode: instructions for use: ¿7.Attach plastic three-way stopcock to the catheter obturator.Fully straighten the curved catheter tip by advancing the catheter obturator. When fully advanced, attach the obturator to the catheter via the luer lock connection.8.Advance the catheter over the wire guide into the pleural cavity to the desired depth.9.Remove the wire guide and catheter obturator.¿ the information provided upon review of device master record, product labeling, device history record, and examination of the returned product does not indicate the device was manufactured out of specification. There is no evidence of nonconforming material in house or in the field.Based on the information provided, inspection of the returned device, and the results of the investigation, cook has concluded that component failure unrelated to manufacturing or design deficiencies contributed to the event. It is possible the wire was damaged during insertion, and then unraveled during removal. It is also possible the patients anatomy contributed to this failure. However, none of these possibilities can be confirmed without additional information. The appropriate personnel have been notified.Cook will continue to monitor for similar complaints.Per the risk assessment no further action is required. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
