E1: initial reporter phone: (b)(6).Device evaluated by mfr.: epic vascular 8x60x120, was received for analysis. The device was received with the safety clip removed and the stent partially deployed on the delivery system. No issues were noted with the stent.A visual examination found no kinks or damage to the outer sheath of the device.A visual inspection found the inner sheath of the device to be kinked at approximately 3mm proximal of the tip bond. This type of damage is consistent with excessive force being applied to the device.A visual and tactile examination identified no issues with the tip of the device. No damage or issues were noted with the handle of the device. The safety lock had been removed from the rack.
