Event or Problem Description
During impaction of the femoral component, the inserter broke.There was no known impact or consequences to the patient.Attempts have been made and no further information has been provided.

(b)(4).This follow-up report is being submitted to relay additional information.One tloc 133 offset inserter assy item# 51-222224 lot# 057777 was returned and evaluated.Upon visual inspection the tip of the device had fractured from the device.There are indentations to the strike plate.A review of the device history records identified no deviations or anomalies during manufacturing.Medical records were not provided.A definitive root cause cannot be determined.The event is confirmed via returned product.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
