Additional Manufacturer Narrative
Section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.Based on a review of available complaint information and an evaluation of the available sample evidence, the following was concluded: the complaint of a damaged dialysis catheter was confirmed; however, the root cause was not identified. The product returned for evaluation was two photographs which depicted a portion of a dialysis catheter. Both photographs depicted the venous and arterial extension tubes. The tubes were fully broken/detached from the rest of the device, which was not visible in either photograph. The breaks in the tubes appeared regular.Details about the fracture surfaces were not visible.Damage was clearly visible in the two submitted photographs; however, inspection of the photographs was insufficient to identify the cause of the damage.Consequently, this complaint is confirmed as ¿cause unknown¿ at this time.Potential contributing factors include sharp instrument contact and tensile stress.A review of manufacturing records could not be conducted as product batch details were not received.Reviews of product design and manufacturing documentation did not reveal any recent changes made to the design, materials, or manufacturing processes of the powertrialysis acute dialysis catheter product line. This evaluation will be updated if additional information becomes available.
