Product complaint # (b)(4).Additional information was requested, and the following was obtained: after further discussion with the account. It appears this situation in question, the tech used one vial of vistaseal (vst10) with a lap tip (vstl35).After the first vial was fully discharged, they needed more product and opened a second vst10 for application.Instead of opening a second lap tip, they tried to reuse the same lap tip from first syringe. Not aware that if they placed the tip on the wrong way they could be trying to push thrombin down the lumen that previously had fibrinogen in it.If this was the case the clot would occur immediately at the luer lock position where the two product would be interacting.I have since in serviced the account and staff members on proper handling of vistaseal. No patient harm was reported.Attempts are being made to obtain the following information. To date no response has been provided. If further details are received at a later date a supplemental medwatch will be sent.1.Did the user experience any alleged quality issue with the dual applicator (vstas1) that was packaged within the vistaseal 10ml kit? 2.The event description stated that during a laparoscopic ventral hernia repair that the applicator tip would not screw on to the device, and product would not dispense. Please clarify if the vstl35 lap tip or vstas1 dual applicator is what would not screw onto the device. This report is being submitted pursuant to the provisions of 21 cfr part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date. This report does not reflect a conclusion by ethicon, or its employees that the report constitutes an admission that the product, ethicon, or its employees caused or contributed to the potential event described in this report. If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.
