Event or Problem Description
It was reported that, during thr surgery, when opening an r3 0 deg xlpe acet lnr 36mm x 62mm, the implant fell onto the floor. It was noticed that the packaging had a split in the internal and external plastic packaging, the plastic packaging was sealed at both ends, however the split was on the side edge of the packaging. The external box appeared to be sealed and unopened. The procedure was resumed, after a non-significant delay, with a s+n back-up device. No injury was reported as a consequence of this issue.

Additional Manufacturer Narrative
H3, h6: the associated device and package was returned and evaluated. The visual inspection revealed the sterile packaging is open on the side causing the implant to come out of the packaging.A review made by the quality engineering team revealed that the part complaint is for a 2014 batch number. The defect appears related to shipping or storage error after manufacturing. The part is sealed on the ends, and the long edges come sealed from the packaging supplier. The opened side is tattered and appears opened through excessive, sudden pressure on the packaging.A review of the production order did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident.A review of complaint history for the part number over the past 12 months and for the batch number based on historical data of the device did not reveal similar events for the listed device.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event.According to the packaging sequence, the device should be placed in the inner pouch, as well as the inner pouch should be place in the outer pouch, both should be sealed, and the locking end of the carton should be closed and shrink-wrapped.At this time, we have no reason to suspect that the product failed to meet any specifications at the time of manufacture.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
