Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary pinnacle poly liner - implant fracture.Surgeon would like head and liner sent off for investigation. The product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the rim of the pinn mar +4 10d 32idx50od has fractured.Additionally, two (2) out of six (6) anti-rotation device tabs (ard) had sheared off, fragments were not returned for evaluation. It is evident that the edge of the liner was loaded by the femoral head and patient body weight while implanted.Machining lines from the manufacturing process were yet visible at the opposite site of the fracture, which would not be as obviously present if the femoral head had been loaded more centrally.Based on the observation of the returned femoral head and the pinn mar +4 10d 32idx50od a dissociation event between the liner and acetabular cup can be identified. There is no indication that a design or manufacturing issue has caused or contributed to the reported event.Although the liner failure cannot be traced to design or manufacturing, a definite root cause cannot be established due to there being multiple factors that may influence eccentric loading.Consideration must be given to all potential influences such as surgical process, patient variables (e.G.Activity level and use), anatomical considerations and patient changes over time.Any conclusions from the investigational input provided have to be placed into context with all other relevant factors. It is highly recommended to the patient to consult with their healthcare professional for further assessment.A manufacturing record evaluation was performed for the finished device [121932150 / j62p54] number, and no non-conformances / manufacturing irregularities related to the malfunction were identified during manufacturing. The overall complaint was confirmed as the observed condition of the pinn mar +4 10d 32idx50od would contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot a manufacturing record evaluation was performed for the finished device [121932150 / j62p54] number, and no non-conformances / manufacturing irregularities related to the malfunction were identified during manufacturing.
