Event or Problem Description
It was reported that the procedure was to treat a 70% stenosed lesion in the anterior tibial artery. The 2.5x200mm armada balloon dilatation catheter (bdc) was used in the procedure; however, the luer connection between the bdc and the inflation device was not aligning properly. There was a leak from the hub and the armada balloon would only hold pressure by firmly holding the inflation device onto the bdc hub during inflation. The same inflation device was used with other balloons in the procedure without any issues. There was no adverse patient effect and no clinically significant delay reported in the procedure. No additional information was provided.

Additional Manufacturer Narrative
Visual and functional inspections were performed. The reported loose or intermittent connection issue could not be confirmed during return analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents and/or complaints from this lot.The investigation was unable to determine a conclusive cause for the reported complaint. There was no damage noted to the device during the inspection prior to use, which suggests a product quality issue did not contribute to the reported difficulties. In this case, it is possible that the luerlock of the inflation device was not properly connected to the hub or possibly over torqued resulting in a loose/intermittent connection, however a definitive cause for the reported difficulties cannot be determined since the complaint could not be confirmed during return analysis. There is no indication of a product quality issue with respect to design, manufacture or labeling of the device.
