Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary : it was brought to my attention the metal back keel handle had broken.I was not in attendance. The handle had been notice of it being broken prior to the case starting by the scrub technician. The metal part of the handle was re-process and used in the case. The case went as planned. There was no delay or injury to the patient. The product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the end cap component has fractured at the center of the pin that secures the cap to the shaft. The observed condition of the device was consistent with a random component failure that may have been caused by exposure to unintended forces. The broken fragment was returned.A dimensional inspection was not performed as it is not applicable to the complaint condition. The sigma unicondylar surgical technique (25m0908 / 0612-05-507) states "use the tibial osteotome to gently remove the bone from the keel slot.Do not impact forcefully as this can cause a break of the posterior tibia".The tip of the osteotomy is nicked, potential cause can be attributed to unintended user error. The overall complaint was confirmed as the observed condition of the sigma hp uni fb keel osteotome would contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the complaint condition.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Device history lot : a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code provided is not a finished goods lot number.H3.
