Event or Problem Description
As reported, during a procedure involving angioplasty of the anterior tibial artery, an advance micro 14 ultra low-profile pta balloon catheter separated at the mid-shaft upon insertion of the device.Another manufacturer¿s 4-french sheath was used during the procedure. The anatomy was calcified, and resistance was encountered upon insertion and removal of the balloon catheter. The balloon was not inflated, as the catheter tore upon insertion. The device was removed over another manufacturer¿s 0.018-inch wire, without conducting a counterclockwise rotation of the catheter.A section of the device did not remain inside the patient¿s body. The patient did not require any additional procedures or experience any adverse effects due to this occurrence.

Blank fields on this form indicate the information is unknown, unchanged, or unavailable.Corrected information: h6 (annex a) summary of event: as reported, during a procedure involving angioplasty of the anterior tibial artery, an advance micro 14 ultra low-profile pta balloon catheter separated at the mid-shaft upon insertion of the device.Another manufacturer¿s 4-french sheath was used during the procedure. The patient did not require any additional procedures or experience any adverse effects due to this occurrence.Additional information was received (b)(6) 2024.The device separated as it was pulled back out of the catheter. The balloon was not inflated, as the catheter would not advance across the lesion.Investigation evaluation: reviews of the complaint history, device history record (dhr), instructions for use (ifu), and quality control procedures were conducted during the investigation.A visual inspection was also conducted during the investigation. The complaint device was returned to cook for investigation.Examination of the complaint device confirmed that the catheter was separated into 3 pieces. The proximal piece measured 79.2 centimeters. The middle segment measured 73 centimeters with some elongation and kinking noted at three centimeters and 11 centimeters from the end.The distal segment measured 72.2 centimeters in length. The balloon was found shriveled up and damaged.A document-based investigation evaluation was performed.A review of the device history record found no relevant non-conformances on the lot.A review of complaint history found no additional complaints for this lot number. The product ifu states ¿if resistance is met during withdrawal, apply negative pressure with a large syringe before proceeding. If resistance continues, remove balloon and sheath as a unit.¿ a review of the device master record (dmr) concluded that sufficient inspection activities are in place to identify this failure mode prior to distribution. The information provided upon review of the dmr, dhr, ifu, the returned device suggests that there is evidence the device was manufactured to specification. There is no evidence of non-conforming devices in-house or in the field.Based on the information provided and the results of the investigation, cook has concluded that the patient¿s anatomy and procedural issues caused this incident. The calcified lesion likely contributed to the resistance encountered during insertion, and when the device was attempted to be removed through the sheath, the resistance encountered upon removal caused the stress on the shaft that resulted in it separating. The risk analysis for this failure mode was reviewed and no additional escalation was required. The appropriate personnel have been notified and cook will continue to monitor for similar events. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.

Event or Problem Description
Additional information was received (b)(6) 2024.The device separated as it was pulled back out of the catheter. The balloon was not inflated, as the catheter would not advance across the lesion.
