A visual inspection was performed on the returned device. The reported material separation (tip and core) was confirmed. The reported difficult to remove could not be tested due to the device condition.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents/complaints from this lot.The investigation was unable to determine a conclusive cause for the reported difficulties.Factors that may contribute to difficulty removing the balloon catheter over the guide wire include, but are not limited to, inner diameter of the catheter, outer diameter of the guide wire, device support, buildup of procedural contaminants, user technique, and/or damage to the catheter or guide wire.In this case, analysis of returned device noted the coils were compressed and misaligned.Misaligned coils would result in reduced clearance with the balloon catheter, possibly contributing to the reported difficulty during removal.Misaligned coils can occur due to interaction with the anatomy or an accessory device.Additionally, the separation of the core and tip was noted during catheter removal. It is possible that manipulation of the catheter, when resistance was encountered, contributed to the reported separation (tip and core). There is no indication of a product quality issue with respect to manufacture, design, or labeling.B5 - describe event or problem updated.
