Additional Manufacturer Narrative
Device history record (dhr) review confirmed that companion caddy s/n (b)(6) was serviced and passed all functional testing prior to being released to finished goods.Visual inspection of external and internal components found no abnormalities.Companion caddy failed functional testing for acceptance at incoming inspection due to broken latch that prevented driver from being removed without assistance. The release plunger assembly did not operate as intended, confirming the reported complaint.Additionally, the plunger assembly was missing lubrication necessary for normal functionality.A known functional release plunger assembly was installed and caddy performed as intended.Failure investigation for this complaint confirmed the reported issue during functional testing. The customer complaint was replicated during testing; the root cause of the reported broken latch was determined to be missing lubrication in the release plunger assembly.Failure investigation identified no test failures or damage that could have contributed to the event.Device was not in patient use at time of complaint. This issue will be monitored and trended as part of the customer complaint process.Syncardia has completed its evaluation and is closing this file.If new or additional information is received in the future, syncardia will file a follow-up mdr.
