Event or Problem Description
Medtronic received information regarding a guidance system. It was reported that the patient had a questionable scan in which the re presentative recommended to not proceed with the case, however, the surgeon asked that they still try. They proceeded to register and got green values. It was noted they were diligent in scrolling through the level verification and did not notate any shift.Nothing seemed amiss throughout the case. However, when they took the final xray, it was noticed all screws had deviated and were not to plan which caused a tear in the dural.Screws were redirected manually with flouro and navigation was aborted.Additional information received from a manufacturer representative indicated that the procedure performed was a l4-s1 posterior spinal fusion using open solera in which the issue occurred intraoperatively. There was a 45 minute delay to the case as the screws were re-directed under flouro and the patient experienced no negative side effects. The patient was reported to be "fine" and neurologically intact. The amount of deviation was reported to have been between 3.5 and 10 millimeters (mm).

H3, h6) software data was returned and analysis confirmed the reported event. The results of analysis concluded that given the relatively consistent deviations observed, it was probable that a patient shift caused these deviations, despite a medtronic representative reporting no observed patient shifts. It was possible that a slight shift occurred.Upon examination of the fluoro images, the pin appeared to be securely in place in the right posterior superior iliac spine (psis).Additionally, according to a medtronic representative, the pin underwent rigidity checks.Furthermore, the log files indicated no indication of excessive force applied to the surgical arm.Codes b01, c13, and d11 are applicable to this analysis.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.
