The impacted product was received by the failure analysis lab on (b)(6) 2024.The investigation of the returned device was completed by the failure analysis lab on (b)(6) 2024.Device evaluation summary: the product was returned to mentor for evaluation.Mentor conducted visual inspection, leak testing, and microscopic examination of the returned device.During visual evaluation no apparent damage or visual anomalies were observed on the returned device.Leak testing was performed, according with mentor procedures, and it revealed a tear (tear a) on the anterior view, and two tears on the posterior view (tears b and c), within areas of siltex cracking, measuring 0.1 cm, 0.3 cm, and 0.8 cm, respectively.Microscopic examination was performed, and the cause of tear a could not be identified.Although no conclusion could be reach on the cause of tear a, the instructions for use contain the following caution: causes of deflation of saline implants include but are not limited to the following events: crease fold failure, intraoperative or postoperative trauma, excessive stresses or manipulations as may occur during normal, daily routines including customary and purposeful trauma such as that which can occur during vigorous exercise, athletics, etc.The evaluation determined that the possible cause of the deflation on tear b and c is consistent with normal wear.Siltex cracking is defined as a material separation occurring in the siltex layer and can eventually progress through the shell of siltex devices.Siltex cracking is ultimately a result of high stresses. As part of mentor¿s quality process, all devices are manufactured, inspected, and released to approved specifications.Additional complaint information monitoring for potential safety signals will be conducted through complaint trending as part of post-market surveillance.Manufacturer¿s reference number: (b)(4).
