Medtronic received a report that branches were inserted neatly while retaining their original state detachment with id-1.Afterwards, when the delivery pusher was pulled, it came in contact with the coil.Detachment at the distal end of the 2nd marker of the microcatheter (2 cm from the microcatheter tip).An attempt was made to push the coil with the delivery pusher, but it stuck in the middle and could not be pushed. The delivery pusher was collected. The microcatheter was removed. The coil tail was in the stent jail space, but it might have been in the branch just like that, so a confirmatory contrast study was performed regularly for about 15 minutes. The procedure was completed because the ischemic condition was not confirmed. There was detachment failure. The physician attempted to detach the coil with the instant detached cycle 1.The physician did not try to detach with another instant detacher. There was no manual attempt at detachment via hypotube. There was no no issue with the instant detacher. No patient symptoms or further complications were reported as a result of this event. The device and any accessories were prepared as indicated in the ifu.The patient was undergoing surgery for treatment of a saccular, unruptured right middle cerebral artery (mca) aneurysm with a max di ameter of 2mm and a 1.2mm neck diameter. It was noted the patient's blood flow was normal and vessel tortuosity was moderate.
