It was reported that the patient presented with a st elevated myocardial infarction (stemi) and the procedure was to treat the left anterior descending (lad) coronary artery. The 4.0x8 mm trek balloon dilatation catheter (bdc) separated and the distal portion remained in the anatomy.A snare was used to retrieve the separated portion. There were no adverse patient sequela. No additional information was provided.

Additional Manufacturer Narrative

A visual inspection was performed on the returned device. The reported balloon rupture and separation were confirmed. The reported difficulty to remove could not be replicated in a testing environment as it was based on operational circumstances.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents and/or complaints from this lot.It should be noted that the coronary dilatation catheters (cdc), nc trek rx, global, instruction for use (ifu) stated: if resistance is felt, determine the cause before proceeding.Continuing to advance or retract the catheter while under resistance may result in damage to the vessels and / or damage / separation of the catheter. There was no damage noted to the balloon dilatation catheter (bdc) during inspection prior to use or leak noted during preparation, which suggests a product quality issue did not contribute to the reported difficulties.Based on the reported information, it is likely that the balloon interacted with the heavily calcified anatomy such that the balloon became damaged and subsequently ruptured during inflation. The difficulty reported during removal was likely the result of the ruptured balloon material catching on the patient¿s heavily calcified anatomy and subsequently force had to be used and the balloon ultimately separated. In this case, it is likely that the combination of the ifu deviation and the reported difficulty removing the device contributed to the reported separation.Additionally, seven millimeters of the distal shaft and the distal half of the balloon separated and became stuck at the lad.No snaring was required, everything was pulled out together as a unit.A new abbott nc balloon was used to complete the procedure. The investigation determined the reported balloon rupture, difficulty removing the device, foreign body in patient and serious injury/illness/impairment appear to be related to operational context. The reported separation appears to be related to use error/operational context. There is no indication of a product quality issue with respect to design, manufacture or labeling of the device.B1 - adverse event/product problem updated from product problem to adverse event/product problem.B2 - outcomes attributed to ae updated from na to required intervention.H1 - type of reportable event updated from malfunction to serious injury h6 - health effect - clinical code updated from 4582 to 2687.H6 - health effect - impact code updated from 2199 to 4614.
