The reported field event of set screw anomaly was confirmed.Final analysis found that the right ventricular setscrew was missing, and the rv septum was damaged upon received. This is consistent with the reported field event of setscrew fell out during operation. The rv septum was damaged during operation could cause the setscrew fell out when it was over loosened. The device was above elective replacement indicator (eri) when received.Telemetry, impedance, sensing, pacing and high voltage (hv) output functions of the device were tested and found to be normal.
