Product complaint: (b)(4).Investigation summary: this instrument is composed of 3 parts. The fem extractor tip is damaged, the femoral impactor is cracked and the femoral introducer tightener is broken, as per the images attached. The product damage documented in this pc has been identified during loan kit inspection, by the loan kit technician. The event date and alert date are the date that the inspection took place. There is no surgeon, procedure, or patient details available. The device associated with this report was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device and photo evidence was reviewed from attachment ((b)(4)).Visual analysis of the returned device found that the femoral extractor revealed that the distal tip was heavily deformed. However signs of breakage were not observed. The observed condition of the device was consistent with a component failure that may have been caused by exposure to unintended forces, such as using excessive force in a prying motion in multiple occasions. The overall complaint was unconfirmed as the observed condition of the femoral extractor would not contribute to the complained device issue.Based on the investigation findings, the potential cause is traced to unintended user error. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code (ah0511 provided is not a valid finished goods lot#.
