Complaint is confirmed.Upon visual evaluation, it was noted that the suture and driver were received. The investigation was performed based on picture provided by customer that shows the bio corkscrew 5.5mm had broken.Functional testing was not performed due to the damage to the device. The most likely cause for the reported failure can be attributed to improper bone prep; misaligned insertion; prying/leveraging the device during insertion.
