Event or Problem Description
Medtronic received a report that this patient had an intracranial aneurysm.During the surgical filling of the spring coil, it was found that the qc2-6-3d could not be pushed. After replacement, the push went smoothly. The coil was stuck in the middle of the catheter. There was no damage observed on the catheter or coil.During the surgery, it was discovered that the qc-7-30-3d spring coil had detached automatically. There was no surgical or medicinal intervention required. The pushwire was not bent or broken. There was friction of difficulty during delivery. The physician repositioned the coil twice. The physician did not attempt to detach the coil. The physician did not rotate the delivery pusher during the procedure.Continuous flush was administered during the procedure. No patient symptoms or further complications were reported as a result of this event. The device and any accessories were prepared as indicated in the ifu.The patient was undergoing surgery for treatment of a saccular, ruptured right anterior communicating artery aneurysm with a max diameter of 4.0mm and a 2.0mm neck diameter. It was noted the patient's blood flow was normal and vessel tortuosity was moderate.

Event or Problem Description
Additional information was received that catheter resistance occurred in the middle section. The coils came out of the introducer sheaths smoothly. The coil did not prematurely detach in the catheter. The catheter was not a medtronic product.

Additional Manufacturer Narrative
Analysis of the axium coil revealed that there are no damages or irregularities were found with the introducer sheath. The actuator interface was found securely attached to the coupler tube. The break indicator was found still intact. There were no evidence of detachment attempts at these locations. No damages or irregularities were found with the pusher. The coin was found still against the lumen stop. The shield coil was found intact. The implant coil was already separated from the detach element and not returned for analysis. The detach element loop was found crushed.A manual detachment was then attempted by breaking the pusher at the break indicator, and the release wire was retracted; detaching the detach element with no issues. No other anomalies could be observed. The axium coil could not be used for resistance testing as the implant was separated from the device.Based on the analysis performed, the customer report of ¿premature detachment" was confirmed. The cause of the premature detachment was found to be due to the implant break/separation from the detach element. It is likely the implant separation occurred due to advancing/retracting against the reported catheter resistance.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
