Event or Problem Description
As reported, during transurethral holmium laser lithotripsy of the kidneys and ureters, the user opened the package of biwire nitinol hydrophilic wire guide on the operating table and prepared to use the wire guide, but discovered that the coating had peeled off.Another same device was used to complete the procedure. No section of the device remained inside the patient's body. The patient did not require any additional procedures due to this occurrence. The complainant has not reported any adverse effects on the patient due to this occurrence.

Additional Manufacturer Narrative
Blank fields on this form indicate the information is unknown, unavailable, or unchanged.Investigation evaluation: as reported, during transurethral holmium laser lithotripsy of the kidneys and ureters, the user opened the package of 'biwire nitinol hydrophilic wire guide' on the operating table and prepared to use the wire guide, but discovered that the coating had peeled off.Another same device was used to complete the procedure. No section of the device remained inside the patient's body. The patient did not require any additional procedures due to this occurrence. The complainant has not reported any adverse effects on the patient due to this occurrence.Reviews of the complaint history, device history record (dhr), instructions for use (ifu), and quality control (qc) procedures were conducted during the investigation.Visual inspection of the returned complaint device was also conducted. One, used, 'biwire nitinol hydrophilic wire guide' was returned for investigation. The coating has two areas of damage to wire guide jacket, with a jagged appearance to damage. The returned complaint device was reviewed by the supplier who noted multiple areas of skived/cut damage to the polymer jacket material, including metallic core wire exposure.Additionally, a document-based investigation evaluation was performed.A review of the device master record (dmr) concluded that sufficient inspection activities are in place to identify this failure mode prior to distribution.A review of the device history record (dhr) found no related non-conformances reported for lot.A complaint history database search showed no other related complaints associated with the failure mode for the complaint device for lot.Based on the available information, cook has concluded that there is no evidence suggesting nonconforming product exists either in house or in the field.Cook has concluded that the device was manufactured to specification.Per the suppliers investigation: a review of the device history records of the specimen device does not present any indication of manufacturing defect or anomaly that could have impacted on the event as reported. The history records indicate this product was final inspection tested at the supplier and was determined to be acceptable. The investigation concluded that the product met specification at the time of shipment. At this time, it is not possible to assign a definitive root cause for the event as reported.Based on the information provided and the evidence presented, it appears that the wire guide was not properly hydrated prior to removal from the dispenser hoop.Cook also reviewed product labeling; the ifu [t_hbwg2_rev1] supplied with the device states the following in consideration of the reported failure mode: 'instructions for activating hydrophilic coating:' 'the hydrophilic coating on the wire guide is activated by immersion in sterile water or sterile saline solution.1.Prior to using the wire guide, fill a syringe with sterile water or sterile water or sterile saline solution and attach it to the flushing port on the wire guide.2.Inject enough solution to wet the wire guide surface entirely. This will activate the hydrophilic coating.- hydrophilic coated wires are very slippery when wet.Always maintain control of the wire guide when manipulating it through any device.- for optimal performance, rehydrate the hydrophilic coated wire guide after exposure to ambient environment or after extended use, replace it with a new hydrophilic coated wire guide.' the complaint device is assembled by a supplier to cook who carried out an investigation in addition to cook.A review of the device history record by cook and supplier found no related anomalies.A review of manufacturing procedures by the supplier found controls to be in place, the wire guide are inspected for defects for the entire length of each wire guide.Based upon the available information and results of the investigation, cook has concluded that the cause for the complaint could not be established. The appropriate personnel have been notified and cook will continue to monitor for similar events.Per the quality engineering risk assessment, no further action is required. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
