Additional Manufacturer Narrative
Product complaint (b)(4) investigation summary according to the information received, ¿it was reported that the broach would not adapt to kincise adaptors and gives interference with the calcar planer. There was no issues. No adverse events reported. There was no surgical delay, nothing further to report¿ the product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the post of the actis broach size 7 was bent and exhibits deep scratches all over the surface.Functional testing performed with a sample broach handle (257000000) revealed that the broach was difficult to assemble and disassemble, most likely due to the observed condition of the post. The reported complaint condition was able to be replicated. The observed condition of the device was consistent with a random component failure that may have been caused by exposure to unintended and excessive forces such as but not limited to impaction forces while the device is not fully seated on the handle and excessive force in a prying motion. The overall complaint was confirmed as the observed condition of the actis broach sz 3 would have contributed to the complained issue.Based on the investigation findings, the potential cause is traced to unintended use error. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
