Event or Problem Description
Medtronic received a report that the axium coil would not detach and was found stretched. The patient was undergoing surgery for treatment of a saccular, unruptured aneurysm in the left posterior communicating artery with a max diameter of 4.7 mm and a 3.5 mm neck diameter. It was noted the patient's blood flow was normal and vessel tortuosity was normal. It was reported that angiography showed that the aneurysm was located in the posterior communicating artery and the size of the aneurysm was 4.25mm x 4.7mm.After the echelon-10 microcatheter was in place, qc-4-12-3d was used for forming hoop.Filling in the order, apb-3-8-3d-es, qc-2-4-3d, apb-1-5-4d-es, apb-1.5-4-hx-es, apb-1.5-3-3d- es, finally filling in the apb-1.5--2d-e coil.Angiography showed that the aneurysm was densely embolized, and the apb-1.5-2-3d coil was implanted for final detachment. However, the coil could not be detached after several attempts.Later, an attempt was made to mechanically detach the spring coil, which was stretched and left in the blood vessel.A thrombectomy stent was used to remove the stretched spring coil from the body. The angiography showed that the aneurysm was densely filled and there were no missing blood vessels. The stretched coil was retrieved back and the surgery was completed. The reported device and any accessory devices were prepared as indicated in the instructions for use (ifu). No patient symptoms or further complications were reported as a result of this event.

Additional Manufacturer Narrative
H3.Product analysis: equipment used: vis (m-78210), 203cm ruler (m-83361) as found condition (condition of returned device): an axium prime coil was returned for analysis within a shipping box; within a sealed biohazard pouch and within a dispenser track. The axium prime coil was returned within introducer sheath.Visual inspection/damage location details: the actuator interface was found to be intact.Upon further inspection the pushwire was found to be kinked at ~3.3cm from proximal end.The coin was found still against the lumen stop. The shield coil was found intact with the implant coil still attached. The implant coil was found to be stretched and damaged. No other anomalies were observed.Testing/analysis (including sem reports): the distal outer jacket was removed, and a manual detachment was then attempted by retracting the release wire and the release wire successfully detached implant coil. Conclusion: based on the analysis performed, the customers report of ¿non-detachment¿ was confirmed as the pusher was returned with the implant coil still attached. However, the implant coil was successfully detached manually during testing. The likely cause for the non-detachment are the bends and kinks found on the pusher, causing the release wire to become stuck. The implant coil was found damaged, and the pusher was found bent/kinked.Possible causes for coil and pusher damage are device was not hydrated, continuous flush was not administered during procedure, advancing device against resistance or damaged during shipment.Based on the device analysis and reported information, the customer¿s report of ¿coil stretch¿ was confirmed. There was no reported issue pushing the axium prime coil out from within the introducer sheath. Therefore, it is possible the axium coil became damaged upon removal from within the introducer sheath; however, the root cause could not be determined.H6.Coding updated based on analysis findings.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
