Event or Problem Description
Information was received from a manufacturer representative regarding a patient who was implanted with an implantable neurostimulator (ins) for unknown indications for use.It was reported that the rep states that the 'curve' was not lined up with the 'little blue fin' on the stylet that they use as a guide to determine how lateral they are going--when they inserted it 'didn't seem aligned'.The rep states they didn't necessarily have issues beyond what was noted here. The rep does have the stylet for this case they will send back.[see similar reported event reported in (b)(4)].

Event or Problem Description
Additional information was received from a manufacturer representative (rep). The rep reported that the stylet and "blue fin" did not seem to align.[see similar reported event reported in 706180200].
