Event or Problem Description
An attempt was made to use an onyx frontier drug eluting stent to treat a moderately tortuous, moderately calcified lesion with 95% stenosis in the proximal right coronary artery (rca). The device was inspected with no issues.Negative prep was performed with no issues. The lesion was pre-dilated.The device did pass through a previously deployed stent.Resistance was not encountered when advancing the device.Excessive force was not used.It was reported that inflation difficulties and a balloon burst occurred during balloon inflation. The balloon was inflated three times to 18, 20, and 24 atm prior to the balloon bust.Inflation difficulties were encountered when the balloon was inflated to 24 atm, and the burst occurred on the third inflation at 24 atm. remnants of balloon material remained in soft tissue after the stent balloon ruptured. The foreign body was removed under local anesthesia and conscious sedation by a surgeon.A piece of the balloon was removed from the soft tissue, and the distal end was frayed. The patient is alive with no further injury.
