Event or Problem Description
One onyx frontier coronary drug eluting stent was implanted to treat a mildly tortuous, mildly calcified lesion with 80% stenosis in the ostium left anterior descending (lad) artery. The device was inspected with no issues.Negative prep was performed with no issues. The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was not encountered when advancing the device.Excessive force was not used during the delivery.Seven weeks post stent implantation the patient came back for another left heart catheterization procedure for recurrent non-st-elevation myocardial infarction (nstemi).During the procedure issues and symptoms were discovered. It was reported that part of the implanted stent had broken off. a polymer potentially came off, and the stent thrombosed. The detached portion of device does not remain in patient. An aspiration catheter was used to remove the stent part that detached. The explanted part has been sent to pathology. The patient is alive with no further injury.
