Event or Problem Description
It was reported to vyaire medical that motor fault occurred on the vela ventilator.The customer reported there was no patient involvement associated with the reported event.

Results of investigation: vyaire medical field service technician went onsite to evaluate the device.Technician replaced mainboard.Motor faul alarm, checked turbine motor printed circuit board and led was off.Replaced power supply, turbine motor led was on and unit was working properly.Vyaire medical will submit a supplemental report in accordance with 21 cfr section 803.56 if additional information becomes available.
