Additional Manufacturer Narrative
The device history record for lot 30207300 was reviewed and the product was produced according to product specifications. The actual complaint product was returned for evaluation.Upon receipt of the device, an obvious separation of bumper sponge from the bumper film was seen. The tubing where the bumper sponge was also seen cut off or separated from the entirety of the tube. The tubing and bumper appeared dirty with the bumper component color was black prior to decontamination, had a mixture of dark brownish-blackish colors. The tubing markings are faded, making it difficult to see the graduation numbers. The area where the bonding of bumper location was viewed and examined under magnification (10x).The bond appears intact. However, when the bumper assembly was inspected, it appears that the film that wraps on a balloon foam burst from under. The film appeared to have jagged edges. Also examined the cutting effects on the tubing, there's one partial cut approximately 1.0 cm then the second cut was the tubing towards the bumper portion.A root cause could not be determined. All information reasonably known as of 10 jun 2024 has been included in this health authority report.Should additional information be obtained, a follow-up health authority report will be provided. The information provided by avanos medical inc.Represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to avanos medical inc.Avanos medical inc.Has no independent knowledge of the reported event but is relaying the information provided by the user facility where the incident occurred. This product incident is documented in the avanos medical complaint database and identified as complaint (b)(4).This information is submitted pursuant to 21cfr803, in compliance with the medical device reporting requirement and should not be considered to be an admission that an avanos medical inc.Product is defective or caused serious injury.
