The device was not returned for evaluation.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to the reported event.Based on the information reviewed, a cause for the reported unrelated mitral regurgitation (mr), heart failure and death could not be determined. It was however, confirmed by the physician that the mr was unrelated to the mitraclip device.Mitral regurgitation, heart failure and death are listed in the instructions for use (ifu) as known possible complications associated with mitraclip procedures. There is no indication of a product issue with respect to manufacture, design or labeling.
