The product was not returned.Product history records were reviewed and the documentation indicated the product met release criteria.Information was provided that indicated the use of a qualified cartridge and handpiece. The company lens model is approved for use in the company b and c cartridge only.A non-qualified viscoelastic was indicated. The root cause was determined to be most likely related to a failure to follow the instructions for use (ifu).A non-qualified viscoelastic was used. The ifu instructs: company foldable intraocular lenses (iols) are qualified for use with a company qualified delivery system (handpiece and cartridge) and ophthalmic viscosurgical device (ovd) combination. The ifu instructs that a company qualified delivery system and viscoelastic combination should be used. The use of an unqualified combination may cause damage to the lens and potential complications during the implantation process. The completed questionnaire indicated that the company current model 18.0 diopter lens was "excised and removed, enlarging the primary wound to 3 mm to facilitate removal." the questionnaire indicated that a new same model company 18.0 diopter lens was inserted without incident. The manufacturer internal reference number is: (b)(4).
