Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary : it was reported that the revision of acetabular liner and femoral head due to dislocation.Light burnishing noted on interior of the shell, due to ceramic contact.Surgeon determined this was not of concern.Old liner and head removed and replaced with new components.Joint restored stable. The product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device and also photos were provided for review.Visual inspection of the returned sample and the provided photos revealed that all anti rotational devices have been sheared off from the pinn mar neut 28idx46od.Based on the observed damage on the liner and the position of the femoral head in relation to the acetabular cup, it is reasonable to conclude that the liner disassociated from the cup. There is no indication that a design or manufacturing issue has caused or contributed to the reported event.Although the liner failure cannot be traced to design or manufacturing, a definite root cause cannot be established due to there being multiple factors that may influence eccentric loading.Consideration must be given to all potential influences such as surgical process, patient variables (e.G.Activity level and use), anatomical considerations and patient changes over time.Any conclusions from the investigational input provided have to be placed into context with all other relevant factors. It is highly recommended to the patient to consult with their healthcare professional for further assessment.A dimensional inspection for the liner was unable to be performed due to post manufacturing damage. The overall complaint was confirmed as the observed condition of the pinn mar neut 28idx46od would contribute to the complained device issue.Based on the investigation findings, the potential cause is traced to component failure, and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot : a manufacturing record evaluation was performed for the finished device 121928046/m2477x , and no non-conformances /manufacturing irregularities were identified.
