Product complaint # (b)(4).The product has been returned for evaluation and testing; however, the engineering evaluation has not been completed.A supplemental report will be submitted if new facts arise which materially alter information submitted in a previous mdr report.Correction to section b5: as reported by the field, during a coil embolization, an orbit galaxy xtrasoft coil (640cx0304, 30763726) filed to detach. The coil was removed. The surgery was delayed by 10 minutes. The procedure was successfully completed.Additional event information received on 07-feb-2024 indicated that a hydraulic test was done.A trufill syringe was successfully used. The coil was attached to the delivery system when removed from the patient. No additional intervention was needed. The coil did not stretch or became damaged when removed. The 10 minute procedural delay was not clinically significant. There are no relevant anatomical information including vessel characteristics.

Additional Manufacturer Narrative
Product complaint # (b)(4).Complaint conclusion: as reported by the field, during a coil embolization, an orbit galaxy xtrasoft coil (640cx0304, 30763726) failed to detach. The 10-minute procedural delay was not clinically significant. There are no relevant anatomical information including vessel characteristics.A non-sterile og cmplx xtrasoft coil 3x4 was received contained in the decontamination pouch.Upon receiving the device, a visual inspection was performed, and it was noted that the device was returned inside the hoop dispenser. One bent condition was noted on the proximal portion of the hypo-tube.No damages were noted on the luer valve it can be properly connected to a lab sample syringe. The coil system could not be removed from the hoop dispenser a strong resistance was felt during the manipulation. The issue reported that the coil was not getting detached could not be evaluated due to the returned condition of the device.Traditional evaluation methods may not suffice in this case due to the unconventional nature of the return. It becomes difficult to ascertain whether the device sustained any damage during post-operation handling and may not guarantee an accurate assessment of the device's functionality. The device was inadvertently kinked during the attempts to be removed from the hoop. The hypo-tube being bent is not originally reported in the complaint and is suggested that the damage occurred during posterior manipulation (i.E.During shipping). This damage is not considered a device failure and therefore not a contributing factor to the issue reported.A manufacturing record evaluation was performed for the finished device 30763726 number, and no non-conformances related to the malfunction were identified. As part of cerenovus quality process, all devices are manufactured, inspected, and released to approved specifications.Devices undergo 100% in-line testing for purgability and flow rate. With the limited information available, no capa activity will be initiated at this time.It should be noted that product failure could be caused by multiple factors. The instructions for use (ifu) does contain the following recommendations: coil detachment must be confirmed under fluoroscopy by releasing the second rhv and slowly retracting the delivery tube ensuring that the coil is not being retracted into the tip of the infusion catheter and that the delivery tube marker bands move away from the coil without resistance. As part of the post market surveillance program, information from this complaint is trended to identify statistical signals for consideration of further correction action.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.A supplemental report will be submitted if new facts arise which materially alter information submitted in a previous mdr report.
